Your first visit at Joint Ventures

Thank you for choosing Joint Ventures Physical Therapy and Fitness to provide your physical therapy care. We are excited to help you get better.  There are some things that you can do before your first visit to be prepared and to help your appointment go smoothly and efficiently. It will speed up the registration process and allow you to maximize the time spent with your therapist.  Below is a check list of things to help get you ready for your first visit.  We look forward to seeing you soon!

Before your first appointment, you will get the following emails from Joint Ventures:

One will contain forms that are helpful for you to fill out and bring to your first visit.  

One will contain information about your insurance benefits.

For your first appointment, there are many things you can do that will help your therapist and our administrative staff, including the following:

Please bring the completed paperwork you received via email, a photo ID, and your insurance card.

If you have seen a doctor, please bring your physical therapy prescription.

Make a list of questions for your physical therapist.  This will help you to organize your thoughts and concerns and will allow you to make the most of your time with your physical therapist.   

Write down a list of your symptoms.  Details related to how and when they began, where they are, what they feel like, what activities or movements make them worse, are they worse based on time of day.  Detailed information will help your physical therapist in the evaluation process.  

Be familiar with your medical history.  While some information may not seem relevant to your injury, it can still be helpful to your physical therapist.

Write down a list of medications, dosage, and the condition for which they were prescribed.

Bring any specific information from your doctor or any imaging and test results/reports (eg. X-rays, MRI, CT scan, etc.).

Make a list of other health care professionals you have seen for the injury (eg. chiropractor, acupuncturist, massage therapist, etc.).

Bring or wear clothing that will allow you to move comfortably and will allow your physical therapist to easily access the injured area.  

Be prepared to answer questions relating to your general health and the condition for which you are seeking treatment.  This will help guide your physical therapist’s evaluation and help him/her select the treatments that will help you get better.
